To begin, navigate to the Patient Trace screen. This screen allows you to search both locally within your organisation and more widely across NHS Wales using the WDS.

Complete the Patient Trace Details

Ensure you enter all required fields on the Patient Trace screen, including the patient's family name, given name, gender, date of birth, and NHS number. Use wildcard searches if you are unsure of spellings or wish to capture broader search results.

Search and Register

Click Find to initiate the search. If the patient is found locally or through WDS, review the details carefully. You may then select the patient to re-register them within your organisation.
